Like he’s come to some horrible realization that what he wants with his whole heart is the same thing he’s being forced to beat, to break, to kill. That he can not possibly be the monster that Naomi wants him to be.
The realization that Dean matters more than the will of Heaven is, in and of itself, slightly terrifying to him. It’s part of that desperate shock in his eyes.

OK SO THIS IS THE SELFISH SHIPPER IN ME BUT WHAT IRRITATES ME THE MOST ABOUT NAOMI:
Every entrance she has, she is cutting off a Dean reaction. LIterally every one. And in this one, I know we would have seen Dean’s eyes sink.
Because I think that the only poorly dealt with thing in this night’s ep was that, while we dealt with the escape for Cas and Sam, we didn’t really deal with Dean’s. We know what it IS, obviously. He’s still reeling from the prospect of losing Sammy and coming back from Purg.
But here he is—losing Cas again.
